The Complex Care Coordination Team (CoNeCT) provides a comprehensive care coordination service to patients in the community with the aim to reduce avoidable hospital presentations.Through community-based case management and care coordination, CoNeCT helps individuals at high risk of poor health outcomes due to complex medical and social needs. These individuals typically have difficulty navigating health and social care systems, often becoming high-intensity users of the system at risk of avoidable Emergency Department (ED) presentations and lengthy unplanned inpatient stays. CoNeCT has demonstrated consistent positive outcomes for individuals engaged with the service, as well as the system by reducing pressure on EDs and inpatient beds.CoNeCT is a metropolitan-wide service with bases in Nedlands, Perth, South Guildford, Armadale, Myaree, Rockingham, and Peel.Successful applicants may be relevant for potential like-to-like positions within the CoNeCT Mental Health Expansion service.This is where jobs become careers, passion finds its purpose, and your voice finds its place. Assesses and prioritises patient welfare needs and provides direct assistance to access benefits and other requirements. Liaises with government and other service providers to provide necessary assistance. Works in conjunction with a multidisciplinary team in providing a comprehensive social welfare service for patients.This is a recruitment pool for permanent full-time, permanent part-time, fixed-term full-time, fixed-term part-time, and casual appointments.We currently have a 0.4 permanent vacancy available in our South Team along with 2 full-time permanent positions available for our North and East Teams with potential for flexibility.Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur, including like positions within the CoNeCT Mental Health Expansion Service.
