About FeldCare Connects

We are a leading provider of home healthcare services, seeking a skilled and self-motivated Physical Therapist to deliver high-quality care to patients of all ages as an Independent Contractor. This position is ideal for professionals who value flexibility and independence in their work.

Responsibilities:
  • Deliver one-on-one treatment to observe, record, and report patient responses and changes in condition.
  • Determine individualized plans of care.
  • Utilize our Electronic Medical Record system for intuitive documentation.
Requirements:
  • Current Physical Therapist license issued by the state Board of Physical Therapy.
  • Able to work with patients of all ages.
  • Completion of an accredited Physical Therapy program.
