Nutrition Specialist

About the Role

As a Registered Dietitian at North East Medical Services, you will be an integral part of our healthcare team. Your primary responsibility will be to provide nutritional counseling services to individuals and families, while also serving as a nutrition consultant to staff and participating in the development of educational materials.

Key Responsibilities:

• Develop and implement age-appropriate, culturally sensitive tools for nutritional counseling on both normal and therapeutic diets.
• Assess patient needs, establish priorities, design, develop, recommend, update, implement, and evaluate programs, curriculum, materials, protocol, procedures, and standards for nutrition education/health education.
• Conduct Medical Nutrition Therapy (MNT) upon physical referrals.
• Document accurately and thoroughly in the Participants' electronic health record in accordance with industry and departmental standards.
• Contribute and assist in the development and periodic updates of the Nutrition EHR templates for charting.
• Implement nutrition care to include follow-up and referrals to government, community and/or other NEMS for continuity of nutrition care.
• Develop meal plans as appropriate to meet members' lifestyle, socio-economic background, age, culture, diagnosis, and religious affiliation.
• Establish relationships with community agencies for patient referrals.
• Utilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
