We connect over 2,000 clinicians with patients in their community by partnering with over 900 home health agencies throughout CA, AZ, TX, and FL. Our goal is to facilitate access to quality care, allowing patients to receive treatment in the comfort of their own homes.

Responsibilities and Qualifications

You will be responsible for delivering one-on-one treatment to patients, developing personalized plans of care, and using our Electronic Medical Record system for documentation. A valid Physical Therapist license, accredited education, and a smartphone are essential requirements for this role.
