Job Description

The Complex Care Coordination Team (CoNeCT) provides a comprehensive care coordination service to patients in the community, aiming to reduce avoidable hospital presentations. Through community-based case management and care coordination, CoNeCT supports individuals with complex medical and social needs, facilitating their access to necessary services.

Key Responsibilities:
Assess and prioritize patient welfare needs, providing direct assistance to access benefits and other requirements.
Liaise with government and other service providers to deliver necessary support.
Collaborate with a multidisciplinary team to offer a comprehensive social welfare service for patients.

Requirements:
Proven experience in social services or a related field.
Excellent communication and interpersonal skills.
Ability to work effectively in a team environment.
Strong problem-solving and critical thinking skills.
