About the Role
The Government of Western Australia Department of Health is seeking a suitably experienced and enthusiastic nurse to fill the position of Clinical Nurse in the community health setting.
The successful applicant will be responsible for conducting home visits and appointments in the Armadale West region as part of a funded project, providing high-quality healthcare services to clients.
Key Responsibilities:
- Conduct thorough assessments and provide care to clients in their homes or in our clinics
- Develop and implement individualized care plans that cater to the unique needs of each client
- Collaborate with other healthcare professionals to ensure comprehensive care and support
- Communicate effectively with clients, families, and colleagues to ensure seamless care coordination
- Participate in quality improvement initiatives and contribute to the growth and development of the team
Requirements:
- Degree-qualified registered nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Minimum 2 years' experience in community health or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Willingness to travel within the designated area
