We are seeking an experienced and enthusiastic nurse to join our community health team in the Armadale West region. In this role, you will be responsible for conducting 12 month and 2-year-old appointments as part of a funded project.

Job Description
  • Conduct routine health checks and provide health advice to children and families
  • Collaborate with multidisciplinary teams to develop and implement care plans
  • Provide education and support to families on healthy lifestyle choices and disease prevention

This is a fixed-term contract for 3 years with the possibility of extension. We are offering a level 2 clinical nurse position with opportunities for professional development and growth.
