Job Title: Care Manager Professional

As a key member of our Home Care team, you will be responsible for delivering and developing tailored care plans that cater to the unique needs and preferences of each client.

Key Responsibilities
  1. You will provide high-quality services tailored to individual clients' needs, ensuring they receive the best possible care within set service expectations.
  2. Conduct ongoing assessments of clients' changing needs and develop care plans in consultation with key stakeholders, including family members and healthcare professionals.
  3. Maintain up-to-date care plans that reflect clients' evolving needs and preferences.
Requirements
  • Certificate IV in Aged Care or equivalent qualification and at least 2 years of relevant experience in the industry.
  • In-depth knowledge of Home Care Packages Program, CHSP services, Aged Care Quality Standards, and Commonwealth Home Support Programme.
  • Experience working effectively in a multi-site environment, with a strong ability to adapt to changing circumstances.
  • A current driver's license and willingness to travel between clients, sites, and services as required.
  • Strong administrative and computer skills, with proficiency in software applications relevant to the role.
